All perfect praise be to Allaah, The Lord of the Worlds. I testify that there is none worthy of worship except Allaah, and that Muhammad, sallallaahu ‘alayhi wa sallam, is His Slave and Messenger.
Allaah The Almighty does not punish His slave for what he has done unintentionally. Allaah The Almighty Says (what means): {And there is no blame upon you for that in which you have erred but [only for] what your hearts intended. And ever Is Allaah Forgiving and Merciful.} [Quran 33:5] Ibn Katheer commented: It means that the sin is shouldered on the one who deliberately does falsehood. Allaah The Almighty Says (what means): {Allaah Does not Impose blame upon you for what is unintentional in your oaths, but He Imposes blame upon you for what your hearts have earned.} [Quran 2:225]
Ejaculation of semen is not a sin for which a man is punished unless he lets it out through prohibited ways, such as masturbation, adultery or the like. In case semen comes out due to a sickness or the like, not by masturbating, then he will not be held accountable for that, nor will he be required to make Ghusl as we previously clarified in Fatwa 82518.
However, it would be preferable to make Ghusl to be on the safe side and avoid the difference of opinions of scholars. That is because some scholars are of the view that Ghusl is obligatory when the semen comes out whether or not it is accompanied by pleasure. In our view in Islamweb, Ghusl is not obligatory after semen comes out due to a disease or the like. You may read the Fatwa we referred you to above which differentiates between semen, pre-ejaculatory fluid and Wady. Perhaps what comes out from you is not semen.
Allaah Knows best.
